FREE CONSULTATION.
We offer a free 15 minute phone or in-person consultation to see if we are a good fit for one another and to get acquainted with our services. This is a good time to get any of your initial questions answered!
INITIAL CONSULTATION.
Before this 60 minute visit, Dr. Lotito will review the extensive intake questionnaire that you must fill out. She will also review any labs which you have provided. During the visit, we will collaborate to create a plan for your journey to optimal wellness. $495
FOLLOW-UP CONSULTATION.
In this 30-45 minute appointment we will review any testing that has been ordered and your progress in Lifestyle changes. This can be an in-person appointment or done via a telemedicine platform for patients who cannot come into the office. $250-350

IS TESTING INCLUDED IN THE COST OF AN APPOINTMENT?
No, specialty testing is not included in the cost of an appointment. We strive to find the lowest cost yet accurate testing available and do not over order testing. We can utilize Labcorp and Quest for many laboratory orders.
CAN YOU BE MY PRIMARY CARE PROVIDER?
No, Functional Medicine and Wellness is a consulting, specialty practice and as such you will need to have a primary care physician and provide us with their name so we can collaborate if necessary.
ARE YOUR SERVICES COVERED BY MY INSURANCE?
Due to the amount of time Dr. Lotito spends preparing for and carrying out each appointment, she does not accept insurance reimbursement. We would be happy to provide you with a superbill you can submit to your insurance for out of network reimbursement.
WHAT IS THE CANCELLATION POLICY?
Due to the time it takes Dr. Lotito to prepare for a visit, 48 hours are required to cancel an appointment. In the unlikely event of a late cancellation, there is also a fee equal to 50% of the appointment fee.
